Key Responsibilities

• Administer medications
• Monitor patient conditions
• Provide wound care and treatments
• Maintain accurate documentation
• May perform administrative duties such as patient navigation, referral follow-up, and care coordination

Requirements

• Active Texas LVN license
• Minimum 2 years experience (LTC, Psych, or Primary Care/Hospital)
• CPR certification
