Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person. A caring environment focused on healing and wellness is created. Nursing practices are based on evidence and current research as well as hospital and department policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Direct patient care activities are coordinated and provided on the unit. These activities include nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also a key responsibility. Patient and family-centered education is provided as necessary. Care coordination and individualized treatment planning are conducted as applicable. Assessment of patients' needs is performed and documented comprehensively according to the RN’s scope of practice and hospital policies and procedures. The patient's culture, age, and developmental level are considered in assessment. Patient risk factors and presenting complaints are identified. Patient health information is obtained from family or others as necessary. Objective and subjective data is collected for comprehensive assessment. Initial admission assessments, if done by other team members, are verified. A problem list is developed, prioritizing patient needs based on input from the inter-professional team when appropriate. Actions and interventions are designed relevant to the patient’s culture and age. Desired outcomes are determined in coordination with patients and families in accordance with medical plans of care. Patient care therapies and procedures are fully implemented with empowerment and clinical autonomy. Procedures, treatments, and processes are explained to patients and families. Actions to alleviate patients' stress and discomfort are initiated. Changes in patient conditions are recognized, responded to, and communicated appropriately. Ongoing assessment and response to procedures and therapies are documented according to hospital policy and standards. Tasks are delegated appropriately, and team members are overseen in the performance of patient care activities. The accuracy and completeness of delegated patient care provided by team members are monitored. Educational needs of patients concerning prioritized problems are identified and addressed according to their level of comprehension. The patient discharge process is facilitated by executing discharge orders timely and coordinating with Case Management and others as indicated. Input into patient/nurse assignments is provided, considering available staffing, unit activity, and patient acuity. Active participation in unit-based activities in support of organizational values and priorities is demonstrated. A positive group dynamic and healing environment is promoted. Nursing excellence is assured through adherence to accepted standards, evidence-based practice, current research, and caring philosophy. Participation in quality improvement activities and monitoring of documentation is actively pursued. A holistic perspective regarding patient care is utilized.
