Physical Therapist needed for a travel assignment in a skilled nursing facility setting in Pembroke, North Carolina. Start date 06/29/2026 for an anticipated 14-week contract. Responsibilities
- Conduct initial evaluations of residents to determine PT needs and establish individualized treatment plans.
- Implement therapy interventions focused on mobility, balance, strength, gait, ADLs, and functional independence.
- Reassess progress regularly; modify plans based on patient response and goals.
- Document patient progress clearly in the medical record; communicate findings and recommendations to the care team.
- Collaborate with physical therapy assistants and other disciplines (nursing, OT, speech therapy) to coordinate care and discharge planning.
- Ensure safety, proper body mechanics, and adherence to infection control and regulatory standards during therapy sessions.

Requirements
- Active Physical Therapist license in North Carolina or eligibility for licensure in North Carolina.
- Graduate of an accredited physical therapy program.
- Ability to work effectively in a skilled nursing facility setting with an older adult population.
- Strong assessment, documentation, and communication skills.
- Willingness to participate in a 14-week travel assignment and to relocate for the duration of the contract.
