About the Company HCA HealthONE Presbyterian St. Luke’s is part of HCA HealthONE, one of the largest healthcare systems in the Rocky Mountain region, serving more than 170 care sites in the Denver metro area. The campus also houses Rocky Mountain Children’s, a dedicated pediatric hospital, making it the only tertiary/quaternary combined pediatric and adult hospital in the region. Job Summary and Qualifications The Pharmacy Clinical Manager is responsible for designing, implementing, and managing the pharmacy clinical programs. This includes formulary management, policy and procedure development, and education across the healthcare spectrum. The manager also assists in managing the pharmacy when the Pharmacy Director is unavailable. Duties Interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system. Accurately compound and dispense pharmaceuticals. Monitor drug therapy regimens for contraindications, drug‑drug interactions, food interactions, allergies, and appropriateness of drug and dose. Provide clinical consultation and clarification to practitioners, suggesting cost‑effective therapeutic alternatives as needed. Maintain the hospital formulary, minimizing non‑formulary procurements, utilizing therapeutic interchanges and promoting rational drug therapy selection. Support participation in daily multidisciplinary and bedside rounds. Identify, design, and analyze drug utilization reviews to promote rational drug therapy and determine action plans based on results. Provide accurate and timely medication information to healthcare staff and patients. Develop, manage, and mentor pharmacy clinical programs such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO conversion, renal dosing, and other programs as appropriate. Develop, manage, and mentor antimicrobial management program (AMP). Document all clinical activities and interventions accurately and completely; mentor and educate staff and provide summary reports. Participate in quality improvement, risk management, and patient safety programs, including National Patient Safety Goals, Core Measures, and HCAHPS medication initiatives. Design, analyze, and report adverse drug reactions, medication errors, and pharmacy quality initiatives. Develop and participate in training and mentorship programs for pharmacists, pharmacy students, residents, technicians, nurses, and physicians. Develop and document staff competency continuously, creating improvement plans when deficiencies are identified. Act as primary preceptor for pharmacy students and/or residents related to clinical pharmacy services. Maintain professional competency to meet current and future facility needs. Develop and manage specialty pharmacy practice programs as related to clinical pharmacy services. Provide basic technical support for automated dispensing machines and other pharmacy computer systems. Intervene proactively in conflicts between pharmacy and medical staff regarding drug therapy issues. Participate in disaster response drills and real‑event response as assigned. Participate in code teams as assigned; obtain and maintain BLS‑ACLS or PALS certification as appropriate. Maintain appropriate controlled substance records and develop processes to meet or exceed diversion prevention and monitoring standards. Comply with all regulatory standards; participate in quality and regulatory reviews and audits. Prioritize work and lead others to comply with facility medication turn‑around time policy and procedure. Supervise and assist pharmacy technicians and support staff effectively. Collaborate with the director to establish pharmacy goals and objectives and implement monitoring. Participate with nursing and medical staff committees as necessary. Assist in staff performance evaluations in conjunction with the pharmacy director and operations manager. Act as manager in charge as assigned. Review physician order sets and other evidence‑based medication use guidelines. Publish the pharmacy newsletter and other written and electronic communications. Review drug class reviews and drug monographs. Manage and monitor the drug formulary. Oversee pharmacy policies and procedures. Provide oversight of staff scheduling. Participate in designing and updating pharmacy computer system programs. Maintain pharmacy dictionaries, adding new drugs to the drug dictionary and Gold Standard formulary Charge Master. Perform other duties as assigned. Adhere to the “Code of Conduct” philosophy and the mission and value statement. Position Requirements Licensure/Certification/Registration: Current licensure by the Colorado Board of Pharmacy as a registered Pharmacist. BLS and ACLS required within first 2 months of employment. Education: B.S. or Doctorate of Pharmacy (Pharm.D.). Experience: Minimum of 5 years of hospital pharmacy experience, completion of PGY‑2 pharmacy residency or fellowship, or specialty BCPS or other certification (e.g., FCCP, FASHP). Judgment: makes decisions based on job knowledge and experience, considers impacted areas, seeks advice where applicable. Computer skills: demonstrates proficiency in Microsoft Office, Meditech, and purchasing vendor applications. Knowledge of policies and procedures: articulates understanding of organizational policies, procedures, and systems.
