Physical Therapist Assistant Manager Job Summary Bertrand Chaffee Hospital in Springville, New York is seeking a dedicated and compassionate Physical Therapist to join our team. Responsibilities Evaluate and treat outpatient population with a variety of physical disabilities and injuries Evaluate inpatient population appropriate for discharge Develop and implement individualized treatment plans to achieve patient goals Conduct physical therapy sessions, including exercises, stretching, and modalities Communicate effectively with patients, families, and healthcare team members Document patient progress and maintain accurate records Participate in interdisciplinary team meetings and contribute to patient care planning Act as liaison with schools for clinical education programs. Assumes responsibilities of Rehab Services Manager in her/his absence.
