Hoag Center for Brain Health, part of Hoag Health is a patient-centered medical center for geriatric patients. It is the continuity hub for every patient in the Center, providing longitudinal follow-up and comprehensive medical support, directing polypharmacy review and prescribing, leading advanced care planning, and managing complex comorbidities across every clinical encounter. All specialist consultative services (behavioral neurology, geriatric psychiatry, palliative medicine, neuropsychology, occupational therapy, social work) report back into the single shared care plan that the Geriatric Physician would oversee.
