Job Description Transplant Coordinators coordinate all aspects of care for both transplant recipients and donors. A coordinator participates in medical, administrative, research and teaching duties. The coordinator is an active member of the multi-disciplinary transplant team. This RN will be working with the Heart Failure and Advanced Heart Failure team in addition to working with LVAD and heart transplant patients Development and implementation of IPD critical Pathways IPD discharge planning Facilitating and managing the use of home care Triage of OPD and IPD medical issues with referral to appropriate health care professionals Communications with insurance companies and referring physicians Quality assurance projects Promoting health maintenance education and follow-up Donor & Recipient Family and patient management Location: Detroit Hours: 630AM-5PM, 4- 10-hour shifts. No weekends or on-call requirements. Benefits: Full-Time Qualifications EDUCATION/EXPERIENCE REQUIRED: Bachelor's degree in nursing preferred.
