What You'll Do:

• Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs
• Develop and implement individualized care plans aligned with regulatory and clinical standards
• Administer procedures including manual therapy, ambulation, therapeutic exercises, modalities, and wound care
• Monitor patient progress, adjust treatments accordingly, and document outcomes
• Collaborate with healthcare team members, patients, and families, providing guidance and support
• Complete discharge summaries and maintain detailed, compliant records
• Participate in team meetings, trainings, and ongoing education
• Supervise and support physical therapist assistants, rehab aides, and students as needed
• Ensure practice aligns with APTA and state practice acts, ethics, and conduct standards

What You Need:

• Valid Physical Therapist license in Oklahoma
• Experience in LTC, rehab, or related settings preferred
• Strong clinical assessment and intervention skills
• Excellent communication and teamwork abilities
