Shift: Day Shift – 5x8 Hours (08:00 AM – 05:00 PM)

Required Experience & Skills:

• Minimum 1 year of skilled home health experience required
• Must have OASIS training and documentation experience
• Infusion therapy experience required
• Skilled home care experience required ( Private Duty only experience not accepted )
• Strong IV skills and wound care experience
• Experience with:
  • General medical diagnoses
  • Heart Failure (HF)
  • COPD
  • Myocardial Infarction (MI)
  • Pneumonia
  • Stroke recovery
  • Post-operative care

Schedule & Coverage:

• Scheduling may vary based on office and patient care needs, including:
• Monday – Friday: 8:00 AM – 5:00 PM
• Monday – Friday: 9:00 AM – 6:00 PM
• Thursday – Monday or Friday – Tuesday schedules
• Alternating weekends required
• Visits and hours are largely self-scheduled based on patient needs

Requirements:

• Active RN license
• BCLS certification required
