In Clinic Phone Nurse/Float (RN/LPN) Full Time Professional Sullivan, IN, US 4 days ago Requisition ID: 1537 Education Graduate of an approved school of nursing Experience/Skills Two years’ experience in physician’s office or related experience (preferred but not required). Background in medical-surgical nursing and immunizations preferred Knowledge of clinical function Ability to work in conjunction with other clinic personnel Knowledge of community health resources for alternate health services Knowledge of community health nursing principles and practices Ability to communicate effectively both verbally and in written reports Required Licenses/Certifications LPN or RN Licensure in the State of Indiana CPR Working Conditions Clean, well-lit working environment ROUTINE RESPONSIBILITIES Essential Functions- Phone Nurse Conducting patient assessments over the phone by asking detailed questions about symptoms & medical history Utilizing critical thinking skills to determine the appropriate level of care needed Strong clinical knowledge and understanding of common medical conditions Provide medical advice based on the assessment Directing patients to the right healthcare provider or service Maintaining accurate documentation of each call Demonstrate excellent communication skills and empathy towards patients Updates the patient’s medical and social history, as appropriate Maintains current knowledge of medications Accurately documenting all patient interactions including symptoms, advice given, and follow up plans in the electronic medical record Schedules patients for further testing as directed by the Provider Secures appointment date and time from specialty referral as directed by the Provider Sends in prescription refills as directed by the Practitioner Performs other duties as assigned Professional demeanor and ability to handle stressful situations calmly Essential Functions-Clinic Float Escorts patients to exam rooms and assesses patients per clinic protocols Obtains and thoroughly documents vital signs, chief complaint Updates medication lists, and verifies allergies Updates the patient’s medical and social history, as appropriate Prepares patient for examination and assists as necessary Ensure that ordered tests results are in the patient record and available for Practitioner review at time of patient examination Assures adequate patient records are maintained and transferred as requested Obtains consent for procedures as directed Collects lab specimens and prepares specimens for transport according to policy Assists provider in procedures including but not limited to dressing changes, pelvic exams, laceration repairs, injections, and aspirations Utilizes appropriate aseptic technique Cleans instruments and prepares them for sterilization according to policy Maintains current knowledge of medications and their correct administration based on age of the patient and current clinical condition Follows the five rights of medication administration to reduce the potential for error Documents in the patient record all medication administered and tests performed in a timely and accurate manner and charges appropriately Formulates a teaching plan based on identified learning needs and involves patient and family in education Knowledgeable in growth and development principles and provides age-appropriate care Schedules patient for further testing as directed by the Practitioner Keeps track of follow-up labs, wellness checks for future patient reminders Maintains knowledge of rules and regulations governing the clinic Sends in prescription refills as directed by the Practitioner Responds to emergency situations appropriately and immediately Call 911 as necessary Performs other duties as assigned Works in all clinics assigned and needs arise Clinic Responsibilities-Phone Nurse Records all phone conversations and information given to the patient in the medical record to provide for clear communication among all clinic staff. Provide medical advice based on established protocols Communicating patient concerns and relevant information to the appropriate healthcare provider for further evaluation or treatment Processing prescription refills as directed by the provider Contact patients to discuss the results of laboratory test or imaging studies Identifying situations that require immediate medical attention and directing patients to the appropriate emergency care facility Participate in professional development and assume responsibility for keeping knowledge of skills current During downtime utilize time wisely by assisting with clinic buckets in the EMR Clinic Responsibilities-Float Nurse Ensures that exam rooms and patient accessible areas are safe and that likelihood of patient injury is reduced Assures that examination rooms are always full and that patients are seen in an orderly and fair manner Ensure that acutely ill or injured patients are triaged and seen first Explains delays and waiting times to patients and families Records all phone conversations and information given to the patient in the medical record to provide clear communication among all clinic staff Maintains knowledge of lab collection policy and procedures Performs quality controls for tests and documents results as per policy Monitors and records oxygen tanks monthly Checks AED monthly. Records activity Performs API QC during the year on all tests resulted at site Checks for outdates on supplies and medications on an ongoing basis Inventories medications upon receiving and disposing. Inventories, orders and manages the VFC medications Documents accordingly Participates in professional development and assumes responsibility for keeping knowledge of skills current Office Responsibilities Answers telephone calls in a professional manner and refers patient problems to Practitioner Interact professionally with patients and families.
