RN Case Management Specialist

We are seeking a dedicated RN Case Management Specialist to coordinate management of patient care, ensuring optimum utilization of resources, service delivery, and compliance with external review agencies. The ideal candidate will provide ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. This position aims to enhance the quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness through integrated functions of case management, utilization review, and discharge planning. Additionally, this role will advocate for patients to address their holistic needs.

Coordinate management of care and optimize resource utilization.

Implement and evaluate comprehensive care plans addressing individual patient needs.

Facilitate discharge planning to ensure smooth transitions and continuity of care.

Collaborate effectively with interdisciplinary teams for holistic patient care.

Serve as a patient advocate, addressing and fulfilling patients' preferences and needs.

Must possess a valid California RN license.

BLS certification through the American Heart Association is required.

Minimum of 2 years of recent RN case management experience, preferably in LTAC or STACH settings.

Experience with MCG and discharge planning across acute care settings.

Strong collaboration skills in interdisciplinary team settings.

This is a full-time position with a schedule of Monday to Friday from 08:30 AM to 05:00 PM.
