- Conduct patient evaluations in both inpatient and outpatient environments, crafting personalized treatment plans for their unique healing journeys
- Implement physical therapy interventions that prioritize mobility, strength, balance, and overall function
- Collaborate with a diverse team of nurses, physicians, case managers, and rehabilitation professionals to elevate patient outcomes
- Guide patients and their families through therapeutic goals, discharge protocols, and at-home exercise routines
- Maintain accurate documentation of evaluations, treatments, and patient progress in compliance with facility policies and regulations
- Contribute to care team meetings and interdisciplinary discussions to ensure seamless patient care coordination
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
- State licensure as a Physical Therapist
- CPR certification preferred but not mandatory
- Specialty certifications (e.g., neurological, geriatric) are advantageous but not essential
- Prior experience is highly desirable
- Ability to effectively treat patients facing complex medical and mobility challenges
- Strong communication and collaboration skills are essential
- Proficiency in utilizing digital charting and clinical documentation systems
