Nurse Practitioner (Cardiology & Primary Care)

Location: Thousand Oaks, CA
Schedule: Part-Time (2–3 Days per Week) with Opportunity to Grow into a Full-Time Position

About the Opportunity

We are seeking a compassionate and motivated Nurse Practitioner to join our growing outpatient practice in Thousand Oaks. This is an excellent opportunity for a provider looking to combine cardiology and primary care while working in a collaborative, patient-centered environment.

The position will begin 2–3 days per week , with the opportunity to expand as the practice continues to grow. We are looking for someone interested in building long-term relationships with patients and becoming an integral part of our team.

Responsibilities

• Evaluate, diagnose, and manage adult patients in an outpatient setting
• Provide comprehensive primary care, including preventive care and chronic disease management
• Evaluate and manage cardiovascular conditions such as hypertension, coronary artery disease, heart failure, atrial fibrillation, hyperlipidemia, and peripheral vascular disease
• Order and interpret laboratory tests, ECGs, and diagnostic studies
• Develop individualized treatment plans and prescribe medications as appropriate
• Educate patients on lifestyle modification, cardiovascular health, and disease prevention
• Coordinate care with specialists and other healthcare providers
• Maintain accurate and timely electronic medical records

Qualifications

Required

• Current California Nurse Practitioner license
• National Board Certification (FNP or AGNP)
• DEA registration (or eligibility)
• Strong clinical assessment and communication skills

Preferred

• Experience in cardiology, internal medicine, or family medicine
• Comfortable managing chronic cardiovascular conditions
• Bilingual (English/Spanish) is a plus
