Position Summary The MDS Coordinator is responsible for completing resident assessments, developing care plans, and ensuring accurate reimbursement in compliance with Medicare and Medicaid guidelines. Key Responsibilities Complete and manage resident MDS assessments, including CAAs and triggersCoordinate interdisciplinary care plan meetings and ensure timely updatesDevelop and revise individualized care plans based on resident needsSupport Medicare/Medicaid reimbursement processes and ensure accurate RUG codingReview clinical documentation to ensure compliance and proper billingLead interdisciplinary care plan team meetingsConduct audits to ensure accuracy, timeliness, and compliance of MDS documentationSupport survey readiness and respond to regulatory requestsTrain staff on RAI process and documentation requirementsMaintain compliance with all federal, state, and facility policies Qualifications Registered Nurse (RN) or Licensed Practical Nurse (LPN) per state/facility requirementsCompletion of an accredited nursing programMDS, care planning, and assessment experience preferred (long-term care experience strongly preferred)Strong attention to detail and knowledge of Medicare/Medicaid guidelinesExcellent communication, organization, and documentation skillsAbility to work collaboratively with interdisciplinary teams Additional Requirements Ability to lift up to 75 lbs and perform physical tasks as neededMust meet all state and federal regulatory requirementsStrong interpersonal skills with energy, professionalism, and teamwork focus Equal Opportunity Employer (EOE)
