Schedule: 08:00 - 17:00 (Monday-Friday; weekend rotation of 1 weekend per month; on call 10 days per month with potential additional coverage for staffing needs)
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: One full year of recent adult home health or hospice experience; first-time travelers are not accepted; CPR and BLS; willingness to travel up to 75 miles from the home clinic; reliable transportation; current driver's license; agency-required liability insurance; active license in hand; proficiency in Homecare Homebase (HCHB) with use within the past six months; proficiency in OASIS
Responsibilities:

• Provide home health nursing services through patient home visits.
• Deliver care for start-of-care, wound care, follow-up visits, and other adult home health or hospice services.
• Complete required clinical documentation using Homecare Homebase (HCHB).
• Attend periodic office meetings and support assignment-related activities.
• Manage patient visits, travel, and clinical responsibilities across the assigned service territory.
