Care CoordinatorIntegrated Services for Behavioral Health (ISBH) is a community-minded, forward-thinking behavioral health organization helping people along the road to health and well-being. We meet people in their homes and communities and help connect them to the resources. We serve Southeastern and Central Ohio with a comprehensive array of behavioral health and other services, working with local partners to promote healthy people and strong communities.OhioRISE is a Medicaid managed care program that provides specialized services for youth ages 0-20 with complex behavioral health and/or substance use needs.The OhioRISE Care Coordinator is responsible for creating, planning, and organizing a team as defined by the family. This position facilitates child & family team meetings that support youth at home, in school, and in their communities with the goal of keeping families together and reducing hospitalizations. The team may include partnering with social service providers, community partners, and the families support systems. The care coordinators role includes leading the team in identifying needs, goals, and action steps to meet the families goals. The care coordinator ensures the youth and families have a voice and choice in all coordinated care and services provided.The pay range for this position is $20.19 - $25.03 per hour based on experience, education, and/or licensure.Essential Functions:Uses a strengths based and solution focused mindset in all job functions and interactions internally and externallyDevelop, lead, and facilitate team meetings for the young person and their identified team.Provides care coordination within the family's home, community, or preferred locationPartners along with the young person, family, natural supports, and other service providers to identify care coordination needs/services to ensure the best outcomes for young people and families.Maintains required documentation to access and maintain OhioRISE benefitsMeets quality and fidelity requirements as defined by ISBH OhioRISE standardsContinually acquires and maintains knowledge of community resources.Attends mandatory in-person and virtual trainings and supervisions which will require travelCompletes all assigned trainings within identified timelines and maintains required certificationsFollows ISBH policies and procedures in maintaining confidentiality, professionalism, and incident reportingUtilizes databases and Electronic Health Record systems for accurate client documentationMaintain compliance with HIPAA, mandated reporting requirements, and professional ethical guidelinesTravel within assigned service areaAll other duties as assignedMinimum Requirements:Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field is requiredThree years with a high school diploma or equivalent; orTwo years with an associate degree or bachelor's degree; orOne year with a master's degree or higherKnowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided at time of employment) is preferred • Two years of experience in a coordinated supportive services or care coordination role that required high efficiency, organization, and attention to detail is preferred • Experience working with people with autism spectrum disorders and developmental disabilities is preferred • Experience in one or more of the following areas is required:family systems,community systems and resources,case management,child and family counseling or therapy,child protection, or,child developmentHave the qualifications and experience needed to work with children and families who are experiencing serious emotional disturbance (SED), trauma, co-occurring behavioral health disorders and who are engaged with one or more child serving systems (e.g. child welfare, intellectual and developmental disabilities, juvenile justice, education) is required • Able to effectively communicate through verbal/written expression required • Ability to use a personal vehicle or company vehicle to provide services in homes, schools, and community settings is required • Ability to maintain confidential information is required • Ability to adhere to all safety, rules, regulations and requirements is required • Ability to bill Medicaid without restrictions is required • A valid driver's license and the ability to operate a motor vehicle is required • An appropriate level of auto insurance coverage is required • Ability to manage deadlines within a fast-paced, high-volume environment is required • Ability to operate in an Internet-based, automated office environment is required • Ability to operate, receive and conduct on-camera video calls and/or meetings in an automated office environment is required • Ability to maintain a minimum of 100mbps for high-speed internet connection is requiredPhysical Requirements:Prolonged periods of sitting at a desk and working on a computer is requiredMust be able to lift up to 15 pounds at times is requiredEnjoy a great work environment with an excellent salary, generous paid time off, and a strong benefits package.Benefits include:MedicalDentalVisionShort-term DisabilityLong-term Disability401K w/ Employer MatchEmployee Assistance Program (EAP) provides support and resources to help you and your family with a range of issues.Our Mission: Delivering exceptional care through connectionOur Values:Dignity - We meet people where they are on their journey with respect and hopeCollaboration - We listen to understand and ask how we can best support the people and communities we serveWellbeing - We celebrate one another's strengths, and we support one another in being wellExcellence - We demand high-quality care for those we serve, and are a leader in how we care for one another as a teamInnovation - We deeply value a range of perspectives and experiences, knowing it is what inspires us to stretch past where we are and reach towards what we know is possibleWe're an equal opportunity employer.
