Weekend Nurse Practitioner (Skilled Nursing) Avir at Beaumont

Nurse Practitioner (Skilled Nursing) Major Responsibilities Act as a Skilled Nursing Facility’s leading provider of internal medicine services on Saturday and Sunday. Collaborate with the facility’s interdisciplinary team and physicians to reduce hospitilizations on weekends. Work with the entire facility’s patient population — short and long-term residents — with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians. Evaluate new-admission visits. Coordinate discharge visits. Evaluate patients exhibiting changes in condition. Provide types of care, including but not limited to: Advanced Care Planning. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Prescribe all necessary medications and treatments. Perform comprehensive physical examinations of patients. Develop and implement patient management policies and procedures. Facilitate referrals to other healthcare professionals and medical facilities. Communicate with attending physicians and/or specialists regarding patient care. Qualifications Advanced Practice Licensure (APRN) with a Texas license. Must have an active license and DEA number. Previous experience in nursing or other medical fields.
