You'll work closely with the attending physician to provide high-quality care, managing patients with both acute and chronic diseases. Your role is vital in ensuring the effective management of those facing debilitating chronic conditions. Throughout the day, you're in constant communication and collaboration with key members of the multidisciplinary team, including the Director of Nursing, Social Workers, and Administrators, fostering a supportive environment that ensures every patient receives the best care possible.OUR MISSION:At MedCap Health, our physician-led organization is dedicated to transforming the clinical landscape of post-acute care.
