Care Transitions Navigator for Seamless Discharges

Job Purpose The Care Transitions Navigator will coordinate activities that promote quality outcomes, patient throughput, and discharge planning while supporting a balance of optimal care and appropriate resource utilization. The Navigator will identify potential barriers to throughput and quality outcomes, minimizing delays in discharge plans. Responsibilities Coordinate activities that promote quality outcomes, patient throughput, and discharge planning while ensuring efficient resource utilization and timely discharge planning. Qualifications Bachelor's degree in Social Work required Master's degree in Social Work preferred LMSW/LBSW, or RN licensed by the Texas Board of Examiners, CCM or ACM preferred 1 year of experience in a health‑related setting Hospital case management experience preferred Ability to work independently, manage time effectively, solve problems, maintain detail orientation, stay organized, multitask, react calmly and objectively in emergency situations, and uphold a high standard of ethical behavior and personal expertise.
