Benefits may vary by employment status Responsibilities Providing 1:1 care for clients following their plan of care at all times, while maintaining dignity and independence. Administering medications, feedings, oxygen, ostomy care as needed per client’s plan of care and assessments. Accurately documenting nursing actions and communication with family, case manager, physicians, and documenting progress toward established goals; informing stakeholders of changes in medical condition and needs. Coordinating or ordering supplies, medications, and feedings to ensure adequate amounts are on hand. Ensuring all patient needs are met in accordance with physician instructions. Qualifications LPN/Lpt/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and state of residence. Valid driver’s license, acceptable driving record, and proof of car insurance. CPR professional certification. 1+ years of nursing experience preferred.
