Principal Accountabilities Evaluate patients and establish a plan of care using current standards of practice. Determine patient functional goals and incorporate them into time‑oriented, measurable team goals. Document evaluations within established time frames and use age‑appropriate language and evaluation tools to establish a problem list. Treat patients according to a plan of care by utilizing evidence‑based knowledge, current standards of practice and techniques. Provide goal‑oriented treatment based on predetermined patient needs. Manage a full case load in a timely manner meeting acceptable productivity standards. Reassess and adjust the patient’s care plan based on changes in status. Engage family/caregivers in the patient’s plan of care and discharge planning. Utilize age‑appropriate techniques and consider cultural and religious preferences in developing the plan of care. Document all patient care including treatments, re‑evaluations, and progress toward goals. Update functional goals as needed, write discharge summaries, letters, and reports professionally, complete outcome and PI measures, and enter all billing data within established time frames. Interface with other professionals and students. Coordinate with the team and patient/family to design a home program and participate in patient care rounds. Make recommendations to other team members as needed to implement the interdisciplinary plan of care. Coordinate with outside agencies for equipment, placement and services, act as a preceptor to new staff and mentor rotating staff. Participate in overall day‑to‑day operations of the department. Conserve all resources, evaluate needs for equipment and supplies, and communicate to the appropriate person. Monitor new referrals, schedule new patients within established time frames, and collaborate with staff at all levels to coordinate care and respond to schedule changes. Maintain a safe environment for patients and staff. Adhere to all safety standards, evaluate safety of all clinical equipment, and respond appropriately to emergency situations. Educate patients and families using age‑appropriate teaching methods and culturally sensitive materials. Evaluate effectiveness of teaching and revise plans in collaboration with patients and caregivers. Hold responsibility for own competency, clinical expertise, practice standards, and performance. Qualifications Master’s degree in speech‑language pathology from an accredited graduate program. NJ state license or eligibility for same (NJ Temp license, for completion of CCC).
