Minimum Requirements

• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent degree required.
• Current State License as a Physical Therapist in PA or ability to obtain before start of assignment required.
• Proof of current CPR (BLS) certification required.
• Current driver's license, reliable transportation, and active vehicle insurance required.
• 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
• Previous home health experience preferred.
• Ability to perform prolonged standing, walking, heavy lifting, bending, pulling, and pushing as part of patient care required.
• Ability to travel locally and work a flexible schedule required.
• Must read, write, and speak fluent English.

• Job Duties: Provide evaluations, develop and revise Plans of Care, document progress notes and phone orders, instruct patients and families on transfers and equipment, order rehabilitative equipment, and collaborate with interdisciplinary teams.
• Skills Required: Knowledge of durable medical equipment; manual dexterity and hand-eye coordination; ability to supervise other care team members when applicable.
