Licensed Practical Nurse (LPN) - Per Diem - The Elms Center

Job Summary:

The Elms Center, a reputable skilled nursing facility in Milford, New Hampshire, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our exceptional team! As an LPN at The Elms Center, you will have the opportunity to provide high-quality care to our residents while working in a supportive and dynamic environment.

Licensed Practical Nurse (LPN) Open Shifts;

• Evening Shift 3:00 P.M - 11:00 P.M Monday - Friday, with On Call once a week and every 5th weekend

Licensed Practical Nurse (LPN) Role:

• Provide compassionate, high-quality care to residents
• Administer medications and perform treatments as directed by healthcare providers
• Monitor patient progress and document conditions
• Assist residents with daily living activities and provide emotional support
• Collaborate with the healthcare team to ensure comprehensive care
• Ensure compliance with all regulatory standards

What We're Looking For:

• Valid Licensed Practical Nurse (LPN) license in New Hampshire
• Strong communication and interpersonal skills
• Ability to work effectively within a team-oriented environment
• Experience in long-term care is preferred, but not required
