Community-Based Care Provide personalized care to clients in their homes throughout Pulaski County and the surrounding community. Meaningful Work Build relationships with clients and families while making a direct impact on their quality of life. KEY RESPONSIBILITIES Conduct initial and ongoing client assessments. Develop, implement, and update individualized care plans. Provide nursing services in accordance with applicable standards and regulations. Communicate with clients, families, caregivers, and members of the care team. Conduct initial and follow-up visits as needed. Maintain accurate and timely documentation and visit records. Participate in quality assurance activities and evaluations. Monitor client needs and communicate changes in condition appropriately. Stay current with applicable regulations, policies, procedures, and technology. Uphold FirstLight Home Care standards and policies. QUALIFICATIONS Graduate of a state-approved nursing program. Active Kentucky Registered Nurse (RN) license required. Minimum of one year of nursing experience. Valid driver’s license and reliable transportation required. Current auto insurance required. Strong organizational, communication, and interpersonal skills. Professional, compassionate, and dependable approach to client care. Proficiency with Microsoft Office, Google Suite, and scheduling software preferred. Current CPR certification preferred.
