Work Environment

Key Responsibilities

• Provide comprehensive evaluations and assessments of patients' physical therapy needs to inform the development of personalized care plans.
• Design and implement tailored therapeutic interventions, consistently monitoring and adapting these plans according to patient progress and feedback.
• Educate patients and their families on therapy techniques, exercises, and the use of therapeutic appliances or devices to ensure effective carry-over outside therapy sessions.
• Actively coordinate with physicians and the healthcare team to manage a cohesive approach to each patient's treatment, prioritizing their safety and well-being.
• Utilize electronic health records (EHR) to document treatment strategies, patient progress, and adjustments to care plans efficiently and accurately.
• Maintain a caseload management system to prioritize patient needs and optimize scheduling, ensuring efficient use of therapy time.
• Engage in community outreach to educate the public on the benefits of physical therapy and enhance awareness of our services.

Patient Population / Client Focus

The primary focus of our Home Care Physical Therapists is to assist individuals recovering from surgery, managing chronic conditions, or seeking to improve mobility and function. You will work with diverse populations, including geriatric patients, adults with post-operative recovery goals, and children with developmental challenges. Your role is integral in assessing and facilitating progress as patients strive to reach rehabilitation milestones.

Qualifications

• Current licensure as a Physical Therapist in New Jersey.
• Bachelor's degree in Physical Therapy; a Doctor of Physical Therapy (DPT) is preferred.
• Basic Cardiac Life Support (BCLS) certification to be obtained within 30 days of employment.
• Strong communication skills and the ability to collaborate effectively with patients and interdisciplinary teams.
• Exceptional problem-solving skills and a patient-centered mindset.

Preferred Experience

• Previous experience in home health care or a clinical setting is advantageous but not mandatory.
• Familiarity with advanced therapeutic modalities and intervention strategies in orthopedic rehabilitation.
• Understanding of patient engagement strategies and techniques to motivate adherence to care plans.

Compensation and Benefits

Flex Well Therapeutics offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
