Registered Nurse, Virtual Clinical Care Progression

Support hospitalized patients through care coordination, clinical review, and discharge planning throughout their acute care stay using virtual technology Operate from Virtua’s centralized command center Collaborate with patients, families, and interdisciplinary teams to promote safe, efficient, and well-coordinated transitions of care Review clinical information and identify potential barriers to discharge Support medication accuracy and reinforce patient and caregiver education Coordinate education, logistics, and transition support for enrolled patients Provide onsite assistance when needed to support a smooth transition from hospital to home Deliver virtual care across multiple acute care locations simultaneously Conduct admission-level reviews to support medication reconciliation Identify and elevate safety concerns, documentation gaps, and coordination needs Perform ongoing clinical and discharge readiness reviews Collaborate with interdisciplinary teams on discharge planning and assess eligibility for programs such as Hospital at Home Review discharge documentation for completeness Reinforce education regarding medications, equipment, and follow-up care Maintain compliance with HIPAA, confidentiality, and regulatory standards Participate in the care coordination team and maintain competency in required virtual care technologies Requirements Minimum of 3 years of recent acute care nursing experience Excellent communication skills Assessment skills Clinical judgement Graduate of an accredited nursing program Current State of New Jersey nursing license to practice as a registered nurse Current CPR certification BLS Valid driver’s license Must be at least 21 years of age and meet insurance requirements Ability to maintain competency in required virtual care technologies Core Competencies Demonstrates expertise in acute care nursing with a focus on care coordination, clinical review, and discharge planning using virtual technology.
