Patient Health Advocate - Hybrid Telehealth
Somatus - Revolutionizing Kidney Care
Somatus is seeking a Patient Health Advocate to support CKD/ESKD patients, coordinating care with physicians and care teams, with hybrid telehealth duties.
You’ll educate provider offices about Somatus programs, conduct patient outreach, document activities in the coordination platform, and assist with care transitions.
Requirements include a high school diploma and 1+ year in care management; Medicare/Medicaid experience preferred; bilingual skills are a plus; U.S. residency required.
