Physical Therapist PT Full Time 15K Sign On Bonus

We are seeking a dedicated Physical Therapist (PT) to deliver home health therapy services. Key Responsibilities Include Performing initial and ongoing assessments of patient mobility, strength, and function. Administering and interpreting tests to set functional goals and outcomes. Creating and updating individualized plans of care, documenting progress, and modifying treatments as needed. Delivering therapy interventions in accordance with physician orders. Collaborating with healthcare team members, including physicians, nurses, and caregivers. Providing discharge planning and patient education for home exercise programs. Supervising supporting personnel such as PTAs and HHAs where applicable. Completing required documentation and assessments, including OASIS when permitted. Qualifications Include Degree in Physical Therapy from an accredited program. Current, unrestricted state license. Valid CPR certification, driver’s license, and auto insurance. Skills and qualities Clinical expertise, effective communication, documentation accuracy, teamwork, and compliance with regulations.
