RN - Case Management

Shift: 8 AM to 4:30 PM; Expectation to work every other weekend and holidays in rotation with hospital employees. Job Description The successful candidate will work in the Psych/Rehab department, providing case management services. This position requires interaction with a diverse range of patients and a need for strong organizational and communication skills to facilitate complex discharge planning. Responsibilities Conduct assessments and develop discharge plans for patients. Collaborate with healthcare teams to coordinate care and services. Utilize EMR systems Meditech and Midas for documentation and tracking. Required Experience / Certifications / Licensure Experience with Midas and Meditech.
