Schedule: Mondays 8 5, Tuesdays 6 3, Wednesdays 10 4, Thursdays 10 6
Hours: 30 per week
Contract Length: 4 weeks (potential extension)

Board certified FNP
Active Illinois NP license
DEA registered in Illinois
BLS certification
At least 2 years of FNP experience
Primary care experience within the last 2 years
Comfortable performing well woman exams
