Physical Therapist (PT) - Long Term Care (LTC)

We are seeking a dedicated Physical Therapist (PT) specializing in Long Term Care (LTC) to join our team. In this role, you will provide comprehensive screening, evaluation, and rehabilitation services for patients with physical functioning disorders or wound care needs.

Job Responsibilities:

• Screen, evaluate, and rehabilitate patients, developing appropriate plans of care that comply with all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence-based practices to inform clinical interventions.
• Administer various rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
• Assess patient progress and adjust treatments in accordance with regulatory and clinical practice requirements.
• Evaluate treatment outcomes to determine effectiveness and make modifications as necessary, including referrals based on patient outcomes.
• Provide consultation and counseling for patients, families, caregivers, and other service providers regarding physical disorders.
• Communicate effectively with supervisors and members of the healthcare team regarding patient progress, barriers to treatment, and alternative treatment plans.
• Compile comprehensive discharge summaries of services provided in accordance with regulatory and clinical requirements.
• Document all regulatory and clinically required information within the electronic documentation system while maintaining accurate patient records in line with company policy.
• Record treatment minutes accurately per regulatory and clinical standards.
• Participate in required meetings such as patient care conferences and rehabilitation meetings.
• Engage in required trainings, in-services, and educational conferences to enhance professional development.
• Acquire necessary Durable Medical Equipment (DME) or supplies as specified by regulatory and clinical practice standards.
• Supervise and support physical therapist assistants, rehabilitation aides, and/or students as necessary.
• Adhere to the standards set forth by the APTA and state-required practice acts, code of conduct, and ethics during professional practice.

Qualifications:

• Minimum of 1 year of experience as a Physical Therapist.
• Experience in Long Term Care (LTC) or Rehabilitation is required.
• Must hold a Massachusetts state license to practice as a Physical Therapist.
