Emergency Department Case Manager

The Emergency Department Case Manager provides level of care assessment and discharge planning and coordination in collaboration with the clinical team and medical provider. The Emergency Department Case Manager strives to promote patient wellness, improved care outcomes, efficient utilization of health services, and minimize denials of payment. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.

Education: Graduate of an accredited school of nursing required. Bachelor's in Nursing preferred. Case Management certification preferred.

License/Certification: Current licensure as a Registered Nurse in the State of New Jersey and BLSHCP Certification required 6 months from date of hire or transfer required.

Experience: Excellent interpersonal skills, critical thinking, written communication skills, and computer literacy required as well as analytical and delegation skills. Prior case management/insurance case management experience preferred. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.
