divh2Internal Medicine And/Or General Practice Nurse Practitioners/h2pNow Hiring - Advanced Practice Nurse/Internal Medicine/General Practice NPs/ppWestern MA/ppSalary Range $125,000 to $135,000 a year/ppThe Advanced Practice Nurse will be responsible for, but are not limited to:/polliConduct visits to patients on palliative care in accordance with local, state and federal regulations./liliCommunicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care./liliFacilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment)./liliConsult with other members of interdisciplinary team to contribute to the patients plan of care./liliPrepare and maintain accurate patient records, charts and documents./liliAssist in development of clinical practice guidelines/standards in support of quality palliative care as needed./liliEnsure collaboration with patients, caregivers, medical directors and members of IDT./li/olpEducation:/ppMasters Degree in Nursing/ppProfessional Experience:/pulliMust have a Nurse Practitioners license in the state of operations/liliMust have, at minimum, ONE year experience in the capacity as a Nurse Practitioner in a hospital, skilled nursing or acute care setting (or similar field)/liliBoard Certified in Internal Medicine, or General Practice/liliMust have a Masters Degree/liliMust have a valid drivers license and a reliable source of transportation/liliMust be willing to travel locally/li/ul/div
