Social Worker 2 - Inpatient Psych: PRN

A Brief Overview Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. What You Will Do Provides psychosocial assessment / intervention to assist patients/families. Provides age specific care to patients/families in all age groups. Develops and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs. Develops plans with attention to individual patient and family goals. Provides linkage to community resources to facilitate optimal care Communicates/collaborates with the work team using a multidisciplinary approach Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
