CVS Health seeks a Case Management Analyst for a field-based role in the Chicago area. You will manage a full caseload of waiver members, conduct in-person home visits, and coordinate care with LTSS teams. Travel up to 75% is required to meet members face to face and ensure contractual outcomes.
