Monitoring & Tracking: Implement care plans, monitor patient progress, and communicate status changes promptly. Care Coordination: Connect doctors, specialists, and families to ensure seamless, well-managed care. Discharge Planning: Partner with the healthcare team to prepare patients for a safe transition home. What minimum requirements you’ll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required. Education: Diploma from an accredited school/college of nursing OR professional licensure at time of hire required.
