Registered Nurse Case Manager - Detroit, MI

This contract opportunity invites registered nurses with recent inpatient experience to join a dynamic environment where you will be instrumental in guiding patients through every stage of their healthcare journey.


Schedule:

• Five 8-hour shifts per week
• Day shifts
• Every other weekend required

Key Qualifications:

• Active RN license
• Minimum 2 years of recent acute inpatient case management experience
• Strong assessment, planning, and care coordination abilities
• Proficient in critical thinking, motivational interviewing, and self-management support
• Excellent communication and advocacy skills, with the capacity to work collaboratively in an interdisciplinary team

Primary Responsibilities:

• Conduct comprehensive biopsychosocial and functional assessments for patients and their families/caregivers
• Develop and implement individualized, patient-centered care plans designed to optimize health outcomes and ensure a positive care experience
• Engage and educate patients and families as integral care team members, ensuring clear advocacy and resource facilitation
• Support patients in overcoming barriers to care and achieving established health goals
• Provide counseling regarding treatment decisions, especially around end-of-life issues and advanced care planning
• Ensure seamless care transitions between settings and advocate for services according to health plan benefit structures
• Collaborate with care team members to manage patients with complex medical or social needs and facilitate interdisciplinary care conferences
• Continually evaluate and modify care strategies based on patient responses
• Participate in process improvement activities to enhance quality and safety, including intervention in cases involving abuse or neglect
• Support departmental initiatives and complete additional duties as assigned
