Compassionate LPN: Primary Care & Care Coordination

FoundCare Inc

Foundcare Inc is seeking a Licensed Practical Nurse (LPN) in West Palm Beach, FL, to provide comprehensive nursing care in a Federally Qualified Health Center. The LPN will perform patient intake, administer medications, support care management for chronic conditions, and educate patients. A strong emphasis on patient-centered communication and cultural competence is required. Qualifications include an active LPN license in Florida and current BLS certification.
