Description: This is a home health assignment. The clinician will be responsible for driving to patients' homes daily. The clinician will also attend periodic office meetings. The average daily caseload will be 5 to 8 patients. The weekly productivity expectation is 30 points. Visit type documentation, travel, and other activities have different point values. Clinicians typically see 5 to 10 patients per day, totaling approximately 6 to 8 points. There are no guaranteed hours in this contract. If patient census is low, the clinician should contact the Account Manager to discuss floating to a nearby clinic for additional hours. Case types may include start of care, wound care, follow-up visits, and other adult home health or hospice services. Mileage is reimbursed at the IRS standard rate, with travel between patient homes being reimbursable. Travel to the first patient and from the last patient is not billable. Candidates must be willing to travel up to 75 miles from the assigned care center. Reliable transportation, a current driver's license, and agency-required liability insurance are required. Local candidates are accepted at the same rate as travelers. The clinician must accept every patient on their schedule regardless of the distance from their previous appointment. Declining patients may result in a cancelation of the contract. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing and follow-up patient care will also result in a cancelation. The clinician must hit weekly productivity points of 30, equating to a full-time workload of approximately 40 hours per week. All overtime must be pre-approved. If the clinician is not receiving enough patients each week to meet their financial needs, they will have the opportunity to float to another nearby clinic to pick up additional patients.
