Geriatric Nurse Practitioner — Autonomous Care Leader

Provider Partners Health Plan (PPHP) seeks an Institutional SNP Nurse Practitioner to deliver coordinated, comprehensive care at designated skilled nursing facilities. You will work with the member’s PCP to provide preventive care, diagnose illnesses, order tests, prescribe medications, and set short- and long-term goals with the patient and family in mind. As a clinical liaison to the interdisciplinary team, you will ensure documentation aligns with PPHP standards and support transitions of #J-18808-Ljbffr Provider-Partners-Health-Plan-
