Responsibilities:

• Monitor patients condition and report any changes to the supervising RN or physician
• Administer medication, wound care, and other skilled nursing tasks as assigned
• Educate patients and their families on disease processes, self-care, and medication management
• Document visits and interventions accurately and thoroughly in compliance with regulations
• Collaborate with multidisciplinary healthcare teams to ensure quality and continuity of care
