Seeking a skilled Registered Nurse to deliver high-quality care in a home health setting. This 13-week full-time contract offers a consistent Monday through Friday schedule, from 8:00 AM to 5:00 PM. The position is field-based, involving daily travel to patient homes, with mileage reimbursement provided. Expect to manage a caseload consistent with 30 points per week, averaging 35 patient visits per day.

Qualifications:

• Active RN license in the state of practice, in good standing
• At least 1 year of recent home health nursing experience (field experience highly preferred)
• Proficiency with Home Care Home Base (HCHB) electronic documentation system
• OASIS assessment experience required
• Reliable transportation, valid drivers license, and up-to-date auto insurance

Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans for patients
• Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, identify and report changes in condition, and ensure communication with care teams and physicians
• Educate patients and families on medical conditions, treatment plans, medications, and preventive health
• Maintain accurate, timely documentation within the HCHB system
• Collaborate with other healthcare professionals, such as therapists and social workers, to coordinate comprehensive care
• Attend case conferences, staff meetings, and participate in required continuing education
• Meet all regulatory and agency requirements for accurate documentation and compassionate patient care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
