HOME HEALTH SUPERVISOR

The Home Health Supervisor is a Registered Professional Nurse or a Licensed Practical Nurse, responsible for the coordination, delivery and quality control of home care services under the direction of the Director of Nursing.

FUNCTIONS:

1. Participates in implementation of policies and procedures.

2. Responsible for the delivery of nursing services through the daily scheduling, coordination, supervision, and training of assigned personnel.

3. Responsible for delivery of therapy services, i.e., PT, ST, OT, and MSW.

4. Advises, counsels, orients, evaluates and supervises designated personnel under the direction of the Director of Nursing.

5. Responsible for supervision of Home Health Aides.

6. Conducts weekly team meetings for discussion and problem solving among direct field staff. Conducts field evaluations of each staff member on a yearly basis.

7. Reviews and investigates the clinical content of the records of patients under his/her supervision to ensure quality standards and regulatory guidelines.

8. Collects all clinical documentation to ensure timely submission and accuracy of billing process including 485s.

9. Responsible for collection and evaluation of nursing statistics including productivity and timeliness of paperwork.

10. Coordinates discharge planning with Home Health Coordinators/Liaisons to insure smooth and accurate delivery of services.

11. Identifies and informs the Director of Nursing immediately of any pending staffing or patient issues requiring his/her intervention.

12. Identifies and responds to nursing needs of the patient through screening of initial intakes and patient-related telephone calls.

13. Participates in agency’s audit and Utilization Review Committee as directed.

14. Performs other duties as assigned.

QUALIFICATIONS:

1. Education: Graduation for an accredited School of Nursing. Must be licensed in the State as a Registered Nurse or Licensed Practical Nurse.

2. Experience: Two or more years’ community health experience. Supervisory experience preferred.

3. Knowledge, Skills and Abilities: Knowledge of caseload requirements, management, and patient care needs. Medicare and community services knowledge required. Awareness and understanding of agency admission policies. Understanding and acceptance of agency philosophy and objectives.

4. Physical: Adequate physical and mental health to carry out the duties as outlined in the job description.

5. Employment: The Home Health Supervisor reports to the Director of Nursing, and is employed by the Administrator under the personnel policies of the agency.
