Job Details: Guadalupe Valley Nursing and Rehabilitation Center - Seguin, TX 78155. Position Type: PRN. Overview The Physical Therapist (PT) conducts assessments and reassessments of rehabilitation needs and potential. The PT also plans, implements, and supervises therapeutic programs which include therapeutic exercises, gait training, range of motion testing and exercise, maintenance therapies, heat treatments, baths, and other activities normally performed by a physical therapist. Responsibilities Conduct assessments and reassessments of the patient’s level of function by applying diagnostic and prognostic procedures. Plan, implement, and supervise an individualized therapeutic program designed to restore physical function, improve range of motion, strength, balance, coordination, endurance, or functional ability. Coordinate and collaborate with the Rehab Director, or design professional, to ensure integration of the therapeutic program in the plan of care. Guide and teach the patient in techniques, exercises, or precautions designed to improve range of motion, strength, balance, coordination, endurance, or functional ability. Observe, record, and report the patient’s reaction to treatment and any changes in the patient’s condition or treatment. Document the extent of the patient’s involvement in decisions affecting his/her care. Instruct other agency personnel and patients’ family members in activities of physical therapy in which they may safely and effectively assist the patient. Participate in patient case conferences to coordinate the patient’s plan of care and promote positive therapeutic outcomes. Participate in continuous assessment and evaluation of the agency via Quarterly Record Review, or the quality Assurance/Improvement Program. Supervise PTA treating evaluated patient and be available for phone conference. Participate in wound care per agency policy and procedure if credentialed wound care Physical therapist. Demonstrate initiative and skills in planning and organizing work. Set and meet objectives and find increasingly efficient ways to perform tasks. Complete work, care, and documentation with accuracy within agency time frames. Require minimal supervision and be self-directed. Demonstrate organizational awareness and commitment by understanding and appropriately applying the chain of command in relation to job position and supervision. Understand and apply the agency mission in relation to own job position. Protect and honor customer and coworker confidentiality. Respect customers’ and coworkers’ right to privacy. Meet attendance and punctuality expectations. Cooperate with requests to meet agency needs. Adhere to agency dress code. Recommend and/or support changes to policies and procedures. Demonstrate knowledge of policies and procedures applicable to own job position. Honor requests of management for interim rules. Maintain agency property, supplies, and equipment responsibly. Maintain a safe and well‑maintained work area. Consider effects of words and actions on others. Utilize listening skills that demonstrate understanding of others’ concerns and motivations. Recognize the influence of beliefs and cultures on behaviors. Work toward resolution of interpersonal conflicts. Offer and provide help when others need information, assistance or direction. Develop cooperation and collaborative work efforts that benefit all involved parties. Assist coworkers when workload allows to meet agency needs. Maintain personal health status required for the job position. Maintain professional licensure/certification. Maintain current personnel file information and provide updates in a timely manner. Set own development challenges and volunteer to learn. Assist with orientation of new personnel. Adhere to infection control and safety policies. Attend agency‑provided in‑service programs to fulfill position requirements. Maintain stable performance and emotions in the face of opposition, pressure, or stressful conditions. Physical Requirements Sight and hearing sufficient to comprehend written and verbal communication. Ability to work a minimum of eight (8) hours per day while driving, standing, sitting, assisting patient movement, writing, and speaking. Qualifications Graduate of a curriculum in Physical Therapy accredited or approved by the American Physical Therapy Association, or the Committee on Allied Health Education of the American Medical Association, or the Council on Medical Education of the American Physical Therapy Association awarding a Bachelor’s degree or higher. Current licensed by the state of Texas to practice as a Physical Therapist. Two years of appropriate experience as a Physical Therapist required; one year experience in a home health setting preferred. Travel and Disruption Home visits and/or hourly patient care daily; periodic office meetings. Must have reliable transportation and agency‑required liability insurance. Must adapt to client status and needs as directed; schedule changes daily due to staffing, client condition, and new clients.
