Care Coordination & Assessment Specialist
LifePoint Health
Sycamore Springs, located in Lafayette, IN, is seeking a Care Coordination Coordinator to support patients seeking psychiatric treatment. You will assess needs, refer to services, and schedule initial assessments, guiding patients through the referral and admission process.
