Nurse Practitioner - PRN

In-Home Health Assessments Team Member

Join a team providing in-home health assessments to improve patient outcomes and quality of care. Responsibilities include:

• Conducting comprehensive assessments of adult and geriatric patients, reviewing medical history, medications, social factors, and health risks
• Administering and ordering point-of-care and lab tests as needed
• Collaborating with physicians, case managers, social workers, and caregivers
• Providing health education, referrals, follow-up, and documentation according to protocols
• Offering services at patients' homes or care facilities

Qualifications:

• Master's Degree or equivalent experience, with active NP licensure and prescriptive authority
• Board certification in Adult, Geriatric, Family, Emergency, or Acute Care
• Current BLS, ACLS, or CPR certification
• Valid driver's license and automobile insurance
• Strong communication and computer skills; ability to work independently
