Responsibilities Assume responsibility for evening, night, weekend, and holiday on‑call duty as scheduled and consistent with agency policies and procedures. Assume primary responsibility for total region census during assigned shift, including assessing, planning, implementing and evaluating phases of the nursing process. Maintain regular communication with nurse supervisors, attending physician, and case manager concerning patient/family care needs and response to interventions. Respond to medical or emotional crisis and make home visits to provide direct care as appropriate. Review referrals and initiate contact with client and family. Conduct a complete nursing assessment based on referral data, client/family interview, and clinical observation of patient health status; demonstrate clinical expertise in symptom management. Develop a plan of care in collaboration with client and all disciplines based on assessment data, document need for services, appropriate discharge planning, short‑ and long‑term goals, and appropriate resource utilization; modify plan of care as conditions change. Demonstrate understanding of payer information to maximize care planning. Initiate and maintain a client record utilizing the agency’s standard documentation system. Make follow‑up visits to assess disease progress and evaluate response to treatment; provide support to client/family using teaching principles to promote and restore health and prevent disease and disability when possible; coordinate care with providers. Teach patients and reinforce client/caregiver education; prepare and support family regarding signs and symptoms as appropriate. Refer, collaborate, and coordinate all client care services; consult with physician regarding client needs and plan of care; direct LPNs in carrying out the established plan of care. Direct home‑care aides and supervise plan of care. Refer client/family to other community resources as needed. Attend and participate in in‑service education programs. Seek consultation and guidance from supervisor. Complete reporting requirements in a timely manner. Actively participate in the improvement process known as H3W to identify service line needs and development. Perform other duties as assigned. Provide skilled nursing care to clients in their homes or community health settings, ensuring delivery of outcome‑based, high‑quality, cost‑effective care. Qualifications Education: Graduate of an associate’s or diploma program in nursing education required; graduate of an NLN‑approved BSN program preferred. Experience: One year of nursing experience or one year in a specialty required; experience in medical/surgery or home care preferred. Licensure, Certification, Registration: Registered Nurse with a license to practice in the state of CT required. Knowledge, Skills and Ability: Computer skills. We take great care of careers.
