The program provides inpatient-level care for patients in the comfort of their home. The Hospital at Home Staff RN is a Field Based Nurse with some virtual nursing in a centralized command center located in Charlestown. This is a fully on site position. Field assignments can be within a 25 mile radius of any BILH hospital.Essential Duties & ResponsibilitiesUtilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the homeNavigating patients and families across the continuum of careMonitoring and responding to biometric data per Hospital at Home Command Center protocolsConducting audio/visual patient care visits as directed by the care team and/or protocolEnsuring all interventions in the patient's care plan are implemented efficiently with a patient centered approachLeveraging the resources and expertise of supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundationAdhering to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based carePromoting professional practice and a culture of safety; willingness to engage in process improvement effortsClinical Roles and ResponsibilitiesProviding protocol driven assessment, care coordination and interventions as appropriate informed by Hospital at Home protocols for diverse patient populations with a focus on oncology, medicine and post- operative careExhibiting sound clinical decision making and critical thinking skillsMaintain and practice in accordance with nursing guidelines per state and hospital regulationsFacilitating admissions to Hospital at Home utilizing Hospital at Home guidelinesServing as a role model for professional nursing practiceUtilizing nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problemsSharing expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertiseSupporting research efforts to advance knowledge and promote evidence-based practicePerforming other duties and responsibilities as assigned by the Senior Director of NursingQuality and Safety ResponsibilitiesEnsure compliance with reporting requirements for Sentinel Events and Serious IncidentsParticipate in quality and safety investigations and root cause analysesTake appropriate action to ensure patient safety as a result of any investigation; andServe as a resource in quality improvement processes and techniqueOrganizational RequirementsMaintains strict adherence to the Lahey Hospital and Medical Center Confidentiality Policy.Incorporates Lahey Hospital and Medical Center Guiding Principles, Mission Statement and Goals into daily activities.Complies with all Lahey Hospital and Medical Center Policies. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
