Physical Therapist - Rehabilitation Services

Job Description

We are seeking a skilled Physical Therapist to effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care. The ideal candidate will develop appropriate plans of care in accordance with regulatory and clinical practice standards.

Responsibilities

• Utilize standardized assessments and evidence-based practices to support clinical interventions.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
• Assess patient progress and adjust treatments accordingly in compliance with regulatory requirements.
• Evaluate treatment outcomes for effectiveness and modify services based on evaluations.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Communicate effectively with supervisors and health team members about patient progress, barriers, and treatment plans.
• Document all regulatory and clinically required information in the electronic documentation system.
• Participate in required meetings and trainings, including patient care conferences and rehabilitation meetings.
• Supervise and support Physical Therapist Assistants, rehabilitation aides, and students.
• Adhere to APTA and state practice acts, ethics, and code of conduct during professional practice.

Required Experience / Certifications / Licensure

• 1 year of Physical Therapy experience required; new graduates with Level II experience are welcome to apply.
• Experience in Long-Term Care (LTC) or Rehabilitation is required.
• Active Oklahoma State License is required.

Schedule

This position offers 8-hour day shifts with no weekend or on-call responsibilities.
