• Hours may fluctuate week to week depending on transportation needs.

Cordova Wellness & Rehab is seeking a compassionate, dependable, and organized Transportation Aide (CNA) to assist residents with medical appointments and transportation needs. This position is ideal for a Certified Nursing Assistant who enjoys working directly with residents while providing safe and reliable transportation services.

Transportation Aide (CNA) Position Summary

As the Transportation Aide (CNA) at Cordova Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:

• Schedule resident medical appointments and coordinate transportation arrangements.

• Maintain an organized appointment calendar and communicate schedules with nursing staff and families as appropriate.

• Safely transport residents to and from medical appointments using the facility's company vehicle.

• Accompany residents to appointments and provide assistance, support, and advocacy as needed.

• Assist residents with entering and exiting the vehicle safely.

• Ensure all transportation documentation is completed accurately and timely.

• Communicate appointment outcomes and follow-up instructions to nursing staff.

• Perform CNA duties as needed while supporting resident care.

• Maintain a clean, safe, and well-maintained company vehicle.

• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

Transportation Aide (CNA) Requirements

• An active CNA Certification for the state of Tennessee

• Current CPR/BLS Certification

• Valid driver's license with a clean driving record

• Must meet company requirements to operate a company vehicle

• Ability to safely drive a passenger van

• Experience in long-term care or skilled nursing preferred
